Reviews the most common conditions to assess and red flags to monitor with the goal of decreasing avoidable hospital re-admission.
Objectives:
- Identify the six conditions identified as potentially avoidable hospitalizations
- Describe the changes / symptoms for these six conditions (pneumonia, UTI, CHF, dehydration, COPD & cellulitis)
- Describe the ten main components of a focused nursing exam for a patient with a changing condition
- Describe how to report and document a change in a patient’s condition to nurse, NP or MD.
